Privacy Policy and Data Protection Guidelines

Facility Policies and Guidelines
1. Admission Policy
1.1. All residents must undergo an initial assessment to determine their care needs and suitability for the facility.
1.2. Required documents must be submitted before admission, including:
Valid IDs
Medical history / recent medical records
Signed Admission Agreement
One-month advance and one-month deposit (if applicable)

2. Payment Policy
2.1. Payments may be made via cash, cheque, or direct bank deposit to FC Homecare Sunbest, Inc.
2.2. Upon signing the contract, the party shall pay One (1) Month Advance.
2.3. Monthly fees must be paid on or before the due date.
2.4. Late payments without prior notice shall incur a ₱200 per day late fee.
2.5. If the party provides immediate notice, a grace period of 7 days will be granted.
2.6. If the party has paid a one-month deposit, a 7-day notice prior to termination is required to process the refund before the next due date.
2.7. If care is discontinued mid-month or an untoward incident occurs, charges for unused days will follow the daily rate of ₱1,900, with a prorated refund of any excess.
2.8 Upon admission, an assessment fee of ₱2,800 is required, covering the initial assessment and doctor’s fee.

3. Care and Service Policy
3.1. Caregivers will provide daily assistance based on the resident's care plan, including:
Bathing and grooming
Feeding assistance
Medication reminders (non-medical)
Mobility assistance
Toileting and diaper change
Vital signs monitoring (if applicable)
3.2. The facility provides a clean, safe, and family-like environment for all residents.
3.3. Emergency cases will be immediately referred to the nearest hospital.
3.4. Families will be informed of any significant change in health condition.

4. Communication Policy
4.1. All communication must be coordinated through the official contact person of the facility.
4.2. Caregivers are not authorized to directly negotiate, argue, or dispute concerns with families.
4.3. Updates regarding the resident’s condition will be relayed to the authorized family representative only.

5. Visitation Policy
5.1. Visiting hours are set by the facility to ensure safety and routine care.
5.2. Visitors must follow hygiene rules, avoid bringing infectious illnesses, and behave respectfully to residents and staff.
5.3. Overnight visits are not allowed unless approved for special cases.

6. Health and Safety Policy
6.1. The facility follows sanitation and infection-control protocols.
6.2. Residents must maintain personal hygiene; families may provide personal toiletries and preferred items.
6.3. Any contagious illness must be reported immediately.
6.4. Smoking, alcohol, or any illegal substances are strictly prohibited inside the facility.

7. Medication Policy
7.1. All medications must be properly labeled and accompanied by a prescription from a licensed physician.
7.2. Caregivers provide reminders only unless the caregiver is medically trained and permitted.
7.3. Families must provide updated prescriptions regularly.

8. Personal Belongings and Valuables
8.1. The facility is not liable for lost or damaged valuables.
8.2. Families are encouraged to bring only necessary items.
8.3. Any brought appliances or equipment must be declared and assessed for safety.

9. Termination of Services
9.1. A 7-day written notice is required before terminating the Admission Agreement.
9.2. If the resident’s medical condition exceeds the care level allowed in the facility, FC Homecare Sunbest reserves the right to request transfer to a more appropriate medical institution.
9.3. Deposits and excess payments will be refunded based on prorated calculations after all obligations are settled.

10. Facility Rules and Conduct
10.1. Respectful behavior toward staff and other residents is required at all times.
10.2. Physical aggression, harassment, or verbal abuse will not be tolerated.
10.3. Damage caused to facility property will be charged to the responsible party.

11. Policy on Fee Adjustments
11.1. Any changes in the rates may occur due to inflation, higher operational costs, or additional required services for the resident.
11.2. The facility will notify families in advance before implementing any rate adjustments.
